In October of 2019, an employee and their spouse were fatally injured after being exposed to high levels of hydrogen sulfide (H2S). This tragic, yet fully preventable, incident did not have to happen. Using the CSB released final report on the Aghorn Operating waterflood station hydrogen sulfide (H2S) incident, this webinar will review the findings of the investigation, probable causes, and key learnings, then discuss how they can be applied to the O&G industry, as well as other industries, that have H2S and non-H2S operations. We will review information from the CSB Staff presentations and the final report issued May 2021.
Takeaways:
- General review of the CSB report.
- Examples of how a safety management system (SMS) contributes to overall worker safety and preventing injuries.
- Understand how various elements within a safety management system (SMS) support other elements in the SMS.
- Discussion of what happens when a safety management system is in place versus not having a safety management system.
