The U.S. Chemical Safety Board (CSB) is demanding access to the Tesoro refinery in Martinez, California, in order to investigate a Feb. 12 chemical spill that resulted in first- and second-degree burns to two workers.
Grand Rock Co. Inc. of Painesville, Ohio has been cited by OSHA for one willful and one serious safety violation after a worker suffered the amputation of four fingers on June 12. The employee was operating an unguarded machine that bends tubes at the Painesville automotive parts manufacturer.
OSHA has cited Houston-based Piping Technology and Products Inc. for four repeat and three serious violations after a worker was injured in June from being struck by a broken die piece on a mechanical press. Proposed penalties total $199,800.
Piramal Glass USA Inc. has been cited by OSHA for 21 safety and health violations after a worker suffered a finger amputation while repairing a machine at the Park Hills manufacturing plant on March 14. Prior to maintenance, the machine was not isolated from its energy source. Proposed penalties total $137,400.
Packaging Corporation of America has history of worker injuries, fatalities

A double worker fatality in 2012 and a triple fatality in 2008 did not result in the abatement of hazards at Packaging Corporation of America facilities; seven of the 30 citations issued recently after a worker was injured on the job were for repeat violations.
An employee of Lindenhurst, NY-based Marley Building Materials was shoveling sand onto a conveyer belt Feb. 19th when his left arm got caught in the conveyer belt. His arm was amputated above the elbow.
An unapproved C-clamp used to attach a load to the hook of an overhead crane slid off a 2,600-pound press brake ram as it was lifted, causing the ram to fall to the ground and pin a worker, resulting in his leg being amputated at the knee.
