A 45-year-old machine operator was fatally crushed while he adjusted a set screw on a rolling mill at Cleveland Track Material on Aug. 21, 2014. An OSHA investigation found that workers were exposed to dangerous rotating machinery parts at the facility that rolls steel for railroad industry use. Seven serious violations have been issued as a result of the investigation.

San Diego CA -- OSHA announced at the National Safety Congress and Expo here in San Diego the preliminary top ten most frequently cited workplace safety violations for fiscal year 2014 (Oct 1 2013 – Sept 30 2014).

Employees at Superior Plastics Extrusion Co. Inc., doing business as Impact Plastics, were exposed to two dozen safety and health hazards, including falls and mechanical hazards, according to federal inspectors. OSHA has proposed penalties totaling $66,789 for violations at the company's Putnam, Connecticut, manufacturing plant.

After providing false documentation and making false representations claiming that previously cited hazards related to hydraulic presses had been corrected, Formed Fiber Technologies LLC has been issued 14 safety citations, including willful and repeat citations, as well as a notice of failure to abate with proposed fines totaling $816,500.

OSHA has cited Sterilite Corp., doing business as Sterilite of Alabama LLC, and Marathon Staffing Corp. Inc., for 13 safety and health violations. OSHA initiated the January 2013 inspection at Sterilite's plastics manufacturing facility in Birmingham as part of its National Emphasis Program on Amputations and regional emphasis program to limit worker exposure to high noise levels. Proposed penalties total $49,000.

 An OSHA investigation following the death of a worker at a New Bedford fish processing plant has resulted in serious safety violations against his employer, Sea Watch International Ltd. 

 Following the crushing of a worker's hand in a 150-ton mechanical power press on Nov. 14, 2013, Superior Roll Forming Co. has been cited by OSHA for four serious safety violations.  

After an employee was struck and killed by an 800+  pound piece of lumber that kicked out of a machine at Tanner Timber Services, the company received five citations from OSHA, including a willful one for failing to protect workers from hazardous, unguarded machines.
