White Cedar Shingles Inc. has been cited for nine safety violations by OSHA after a worker was fatally injured May 21 while servicing machinery that had not been locked out to prevent unexpected startup.

According to OSHA, energy sources including electrical, mechanical, hydraulic, pneumatic, chemical, thermal or other sources in machines and equipment can be hazardous to workers. During the servicing and maintenance of machines and equipment, the unexpected startup or release of stored energy could cause injury to employees.

Piramal Glass USA Inc. has been cited by OSHA for 21 safety and health violations after a worker suffered a finger amputation while repairing a machine at the Park Hills manufacturing plant on March 14. Prior to maintenance, the machine was not isolated from its energy source. Proposed penalties total $137,400.
