After you've picked the low-hanging fruit...
Improving safety becomes more difficult

What are two practices that characterize companies with the best safety records?
What I've seen from my experience traveling all over the world are the best practices of employee-driven hazard recognition and behavioral-based peer-to-peer observations.
When a company has already picked the "low hanging fruit" by clarifying safety policies, developing a competent safety office, and establishing proper guarding, they next step is much more difficult. It involves a culture change where employees are willing and equipped to report hazards in the workplace and honestly offer peers feedback on their safe and at risk behaviors.
