DuPont Sustainable Solutions (DSS) is launching a new holistic solution to managing process safety through an integrated, electronic platform, developed in partnership with Houston-based software company, Operational Sustainability, LLC™ (OS).
A Florida construction company employee suffered severe burns to his legs and feet after becoming tripped in hot liquid asphalt – an incident which earned his company OSHA citations for ten serious safety and health violations. Proposed penalties are $63,360.
An explosion in June at a Louisiana gas company that killed two workers and injured 80 people has resulted in $99,000 in fines for the company. Williams Olefins LLC in Geismar, La. was cited for six process safety management standard violations, including one willful.
OSHA has cited Welco Acetylene Corp. for 19 serious safety violations following an inspection of its repackaging facility in Newark. The inspection began in May under OSHA's national emphasis program for process safety management for covered chemical facilities. Proposed fines total $49,200.
The Occupational Safety and Health Administration division (OSHA) within the Wyoming Department of Workforce Services (DWS) has cited the Sinclair Wyoming Refining Company with $707,000 in fines for 22 violations found at the company’s Sinclair, Wyoming refinery operation.

Nebraska Cold Storage Inc. has been cited for 14 safety violations and fined $132,800 by the U.S. Department of Labor's Occupational Safety and Health Administration for exposing workers to anhydrous ammonia at its Hastings facility.

Fox Valley Systems Inc. of Cary, Ill. has been cited for multiple safety violations following an explosion and fire that resulted in serious injuries to three employees on March 6 at the Cary plant. OSHA cited the company for 26 safety violations, including two willful violations where locked doors impeded exit routes and snow blocked exits, slowing employees from exiting the plant quickly.
If OSHA follows through with its recently unveiled new regulatory agenda, employers can expect several changes to the Process Safety Management and Flammable Liquids (PSM) standard that will likely affect their operations, according to Arent Fox, a Washington, D.C. law firm that handles occupational safety and health issues.
