OSHA inspections following an April 14, 2014, incident in which an employee lost parts of two fingers in an inadequately guarded machine have resulted in citations for 16 serious violations against West Hartford Stairs and Cabinets.

Employees at Matalco US Inc. in Canton were exposed to amputation, fall hazards and unsafe crane operations, according to an OSHA inspection which resulted in two willful, one repeat and two serious safety violations, carrying proposed penalties of $130,200.

Workers at Diversified Fabricating, Inc. were exposed to iron oxide fumes at levels higher than the permissible exposure limit and a host of other hazards, earning the Oneonta, Alabama company 23 OSHA citations and $73,000 in proposed penalties.

After providing false documentation and making false representations claiming that previously cited hazards related to hydraulic presses had been corrected, Formed Fiber Technologies LLC has been issued 14 safety citations, including willful and repeat citations, as well as a notice of failure to abate with proposed fines totaling $816,500.

A manufacturer of industrial machinery has been cited for repeatedly exposing workers to amputation and other serious hazards at its Hudson, Ohio facility.

Workers at a Texas manufacturing company were exposed to struck-by hazards and amputation hazards, according to OSHA, which has cited EICA Industries Inc. of Fort Worth for 17 serious violations, with proposed penalties of $46,000.
