A construction company in American Samoa that routinely neglected to ensure workers were anchored or tied off to body harnesses came in for scrutiny by OSHA after a worker suffered a fatal fall in May.

Earlier this year OSHA cited Kamps Inc. for 10 alleged safety and health -- including one willful -- violations at its Versailles wood pallet manufacturing facility. OSHA's inspection was initiated on Nov. 4, 2011, under the agency's Site-Specific Targeting Program, which expands the scope of a routine inspection to cover all hazards associated with the employer's industry. Proposed fines total $101,000.

OSHA has cited Home Depot Inc. for eight safety violations, including seven repeat, carrying proposed penalties of $150,700 after an inspection at its Reynoldsburg, Ohio store. The inspection was initiated under the site-specific targeting program that directs enforcement resources to workplaces where the highest rates of injuries and illnesses occur.

OSHA has cited N.E.J. Abatement Group Inc. for six serious violations involving lead hazards at a Pittsburgh work site. An April inspection was prompted by a referral from the Pennsylvania Department of Health, and conducted by OSHA under its National Emphasis Program on Lead.

In 2004,OSHA issued citations and proposed penalties to Pieper Electric, Inc., of Milwaukee, Wis., for failing to protect workers from electrical hazards. The penalty and OSHA citations are based on an inspection initiated in September 2003 following an incident at a Racine, Wis., wastewater treatment facility where Pieper Electric was contracted to furnish and install various electrical equipment.

OSHA cited O'Connell Electric Co. Inc. of Victor, N.Y., for 14 alleged serious violations of safety standards after a company employee was burned in an electrical arc flash on the North Campus of the State University of New York (SUNY) Buffalo. The electrical contractor faces a total of $88,200 in proposed fines.

The large penalties levied against an Albany, NY manufacturer “reflect the breadth and severity” of the hazardous conditions found at its plant, according to OSHA’s Kimberly Castillon. "The fact that a catastrophic incident has not occurred does not absolve this employer of its responsibility to reduce and prevent risk and eliminate hazards that could injure or kill its workers."

OSHA has cited A & B Foundry & Machining LLC with 33 health and safety violations, including four repeat. The Nov. 15, 2012, inspection was initiated under the national emphasis program targeting the primary metals industry. Proposed fines total $170,107.
