Central Ready Mix LLC has been cited for 10 serious safety violations by the U.S. Department of Labor's Occupational Safety and Health Administration after a 39-year-old plant operator was fatally engulfed in a fly-ash storage silo on Aug. 6 at the Middletown gravel company.

The owner of a New England munitions manufacturing company is heading to prison after being convicted on two counts of manslaughter in the deaths of two of his employees. Craig Sanborn was sentenced in Coös County Court in New Hampshire to five to ten years in prison in the May, 2010 explosion at his Black Mag LLC plant.

A construction company in American Samoa that routinely neglected to ensure workers were anchored or tied off to body harnesses came in for scrutiny by OSHA after a worker suffered a fatal fall in May.

The fatal June, 2013 collapse of a four-story building in Philadelphia has resulted in OSHA violations against the contractors hired to demolish the building. Griffin Campbell, doing business as Campbell Construction, and Sean Benschop, doing business as S&R Contracting, were cited for three willful per-instance violations, following the incident, which killed six people and injured 14.

This is the time of year when OSHA announces their top ten citations of the past fiscal year.  There are few changes in this top- ten list year after year.  Even though the list is of the complete standard’s name, it is usually only one or two sections of a standard that repeatedly makes this list. 

Earlier this year OSHA cited Kamps Inc. for 10 alleged safety and health -- including one willful -- violations at its Versailles wood pallet manufacturing facility. OSHA's inspection was initiated on Nov. 4, 2011, under the agency's Site-Specific Targeting Program, which expands the scope of a routine inspection to cover all hazards associated with the employer's industry. Proposed fines total $101,000.

Nebraska Cold Storage Inc. has been cited for 14 safety violations and fined $132,800 by the U.S. Department of Labor's Occupational Safety and Health Administration for exposing workers to anhydrous ammonia at its Hastings facility.

OSHA has cited seven Trade Fair Supermarkets stores in Queens for 40 violations of workplace safety standards. The local grocery store chain faces $128,000 in proposed fines following inspections that began in March in response to complaints.
