A 25-year-old working on a six-story residential project at Florida State University died after being struck and crushed by a material/personnel elevator carriage not enclosed on all four sides. After an investigation of the July 28, 2014 fatality, OSHA found that Miller's Plumbing and Mechanical Inc. allowed a window-frame opening in a building under construction to be uncovered, thus exposing workers to the hazard of being struck and crushed by the elevator carriage as it passed within inches of the opening.

Raul Saucedo, who had not been provided with high-visibility clothing* by his employer, was fatally struck by a car while cleaning outside the Surlean Foods facility, OSHA has found.

A 54-year-old worker at a Greeley, Colorado was fatally injured in June, 2014 when his hair and arm were caught in an unguarded conveyor system. His employer, JBS USA LLC, failed to protect workers from moving machine parts by properly guarding or de-energizing the equipment, an inspection by OSHA found.

A 21-year-old machine operator was killed at Ventra Belvidere LLC while reaching into a machine to align parts made for Chrysler vehicles. Following an investigation into the June 10, 2014 incident, OSHA, OSHA cited the Belvidere, Illinois facility for one willful and four serious safety violations for exposing workers to dangerous machinery and other hazards.

Two workers were injured, one fatally, after being struck-by an excavator bucket while installing stormwater drainage in a trench in Holdrege, Nebraska on July 17, 2014. OSHA cited Van Kirk Sand and Gravel, which operates as Van Kirk Brothers Contracting, for two serious safety violations related to the incident.

The deaths of two workers in a crane tip-over April 12, 2014, in Bourne, Massachusetts could have been prevented if their employer, Massachusetts Bay Electrical Corp., had set up and operated the crane according to the manufacturer's instructions and trained employees in its proper operation, an inspection by OSHA found.

A 46-year-old worker at a Boston steel fabrication shop died on Dec. 9, 2013, when a 12,000-pound steel bridge arch beam that he was spray painting fell and crushed him. An investigation by OSHA found that his employer, Boston Bridge & Steel Inc., failed to ensure that the fallen beam and three similar beams were adequately braced or supported to prevent them from falling while workers painted them.

OSHA has cited First Choice Energy of Minot with nine serious safety violations for exposing workers to unsafe conditions at an oil field drilling and fluid disposal operation in Stanley. The inspection was prompted after a worker was killed after being caught in the agitator of an oil field vacuum truck storage tank on March 14.
