The National Transportation Safety Board (NTSB) has issued four Safety Alerts to general aviation pilots and mechanics highlighting safety issues identified in several recent accident investigations.
The US Chemical Safety Board (CSB) released a safety message yesterday to commemorate the tenth anniversary of the accident at the BP refinery in Texas City, Texas, that killed 15 workers and injured 180 others. In the message Chairperson Rafael Moure-Eraso emphasizes the need for continued safety improvements across industry in order to prevent similar accidents from occurring.

Chairman of the Chemical Safety & Hazard Investigation Board (CSB) Rafael Moure-Eraso says he’ll leave the board when his five-year term ends in June. In investigating chemically-related industrial accidents, CSB personnel seek to discover the root cause of events and, when feasible, identify and prevent similar problems that other companies will face in the future.
Safety bulletin notes five key lessons to prevent hydraulic shock

Today the U.S. Chemical Safety Board released a safety bulletinintended to inform industries that utilize anhydrous ammonia in bulk refrigeration operations on how to avoid a hazard referred to as hydraulic shock.
The National Transportation Safety Board (NTSB) has extended the comment period for its Notice of Proposed Rulemaking governing the agency’s investigation procedures. The new deadline for comments was moved from October 14 to October 31, 2014.
